Mich. Admin. Code R. 325.3217 - Proper presentation of certificates of death for physician signature
Rule 17.
(1) A
physician responsible for completing the medical certification on a certificate
of death shall provide the certification when a certificate of death is
presented to that individual with at least all the following information:
(a) The name of the deceased.
(b) The sex of the deceased.
(c) The county, city, village, or township
and address of the deceased.
(2) A record completed in the manner
described in subrule (1) of this rule is considered properly presented for the
purpose of obtaining the medical certification.

Rule 17.

(a) The name of the deceased.
(b) The sex of the deceased.
(c) The county, city, village, or township and address of the deceased.
(2) A record completed in this manner is considered properly presented for the purpose of obtaining the medical certification .
