Mich. Admin. Code R. 338.11103 - Identification, written consent
Rule 1103. At the inception of care for a patient, both of the following must occur:
(a) Each
dentist, dental therapist, UDA, RDA, and RDH shall identify themselves to the
patient as a dentist, dental therapist, UDA, RDA, or RDH.
(b) The patient is provided with a written
consent for treatment.
