Mich. Admin. Code R. 338.2121c - Relicensure requirements
Rule 21c.
(1)
An applicant whose cosmetologist or specialty license has lapsed for 3 years or
less after the expiration date of the last license may be relicensed under
section 411(3) of the code, MCL 339.411, after submitting a completed
application on a form provided by the department and the required
fee.
(2) An applicant whose
cosmetologist or specialty license has lapsed for more than 3 years after the
expiration date of the last license may be relicensed under section 411(4) of
the code, MCL 339.411, after submitting a completed application on a form
provided by the department, the required fee, and satisfying either of the
following requirements:
(a) Pass the
examinations required for the applicable license pursuant to Table 21a under R
338.2121a within a 1-year period immediately preceding the date of the
relicensure.
(b) Establish that he
or she has held an active cosmetology license in good standing in another state
within the 3 years preceding application for relicensure.
