Mich. Admin. Code R. 338.7122 - Prescription
Rule 22.
(1) A
prescription must include all the following information:
(a) The name of the patient.
(b) The patient's medical
diagnosis.
(c) The signature of
either an individual that is licensed and authorized to prescribe physical
therapy in this state or an individual that has an equivalent license issued by
another state, as provided in section 17820(1) of the code, MCL
333.17820.
(d) The date that the authorized licensee
wrote the prescription.
(2) A prescription is valid for 90 days after
the date that the authorized licensee writes the prescription unless the
authorized licensee specifies a different termination date on the
prescription.
