Mich. Admin. Code R. 408.12413 - Reports of injuries
Rule 2413. An employer shall, within 30 days of the occurrence, report to the director of the Michigan Occupational Safety and Health Administration (MIOSHA), 530 West Allegan Street, Lansing, Michigan 48933, all point of operation injuries or injuries within the confines of the die to operators or other employees. All of the following information shall be included in the report:
(a) Employer's
name, address, and location of the workplace and establishment.
(b) Employee's name, injury sustained, date
of injury, and the task being performed such as operation, setup, maintenance,
or other tasks.
(c) Type of clutch
used on the press such as full revolution, part revolution, or direct
drive.
(d) Type of safeguard being
used such as 2-hand control, 2-hand trip, pull-outs, or other safeguards. If
the safeguard is not described herein, give a complete description.
(e) Cause of the accident such as repeat of
press, safeguard failure, removing stuck part or scrap, no safeguard provided,
no safeguard in use, or other causes.
(f) Type of feeding such as manual with hands
in dies or with hands out of dies, semiautomatic, automatic, or other types of
feeding.
(g) Means used to actuate
press stroke such as foot trip, foot control, hand trip, hand control, or other
means.
(h) Number of operators
required for the operation and the number of operators provided with controls
and safeguards.
