Subpart
1.
Records of authority and responsibilities; radiation
protection programs.
A licensee must retain:
A. a record of actions taken by the licensees
management according to part 4731.4405, subpart 1, item A, for five years. The
record must include a summary of the actions taken and a signature of licensee
management;
B. a copy of the
authorities, duties, and responsibilities of the radiation safety officer, as
required under part 4731.4405, subpart 1, item E, and a signed copy of the
radiation safety officers agreement to be responsible for implementing the
radiation safety program, as required under part 4731.4405, subpart 1, item B,
for the duration of the license. The records must include the signature of the
radiation safety officer and licensee management; and
C. for each associate radiation safety
officer appointed under part 4731.4405, subpart 1, item B, the licensee shall
retain, for five years after the associate radiation safety officer is removed
from the license, a copy of the written document appointing the associate
radiation safety officer signed by the licensees management.
Subp. 2.
Protection program
changes.
A licensee must retain a record of each radiation protection program change made under part 4731.4405, subpart 2, for five years. The record must include a copy of the old and new procedures, the effective date of the change, and the signature of the licensee's management that reviewed and approved the change.
