Minn. R. agency 144, ch. 4731, MEDICAL USE OF RADIOACTIVE MATERIAL, pt. 4731.4519 - SPOT CHECK RECORDS; TELETHERAPY UNITS
A. A licensee must retain a record of each
periodic spot check for teletherapy units required under part 4731.4472 for
three years. The record must include:
(1) the
date of the spot check;
(2) the
unit manufacturer's name;
(3) the
model number and serial number for the teletherapy unit;
(4) the model number, serial number, and
identity of the source;
(5)
instruments used to measure the output of the teletherapy unit;
(6) an assessment of time linearity and
constancy;
(7) the calculated
on-off error;
(8) a determination
of the coincidence of the radiation field and the field indicated by the light
beam localizing device;
(9) the
determined accuracy of each distance measuring and localization
device;
(10) the difference between
the anticipated output and the measured output;
(11) notations indicating the operability of
each entrance door electrical interlock, electrical or mechanical stop, source
exposure indicator light, and viewing and intercom system and doors;
(12) the name of the individual who performed
the periodic spot check; and
(13)
the signature of the authorized medical physicist who reviewed the record of
the spot check.
B. A
licensee must retain a copy of the procedures required under part 4731.4472,
subpart 2, until the licensee no longer possesses the teletherapy
unit.
