18 Miss. Code. R. 17-5.9 - Proof of Identity (parent)

Acceptable forms include:

1) One (1) Photo ID for the applicant:

a. Driver's license, or

b. Other state issued photo ID, or

c. Passport or other government issued photo ID

2) If no Photo ID available, must submit two (2) forms of identity verification for the applicant.

a. Work or school ID

b. ID for health benefits

c. Voter registration card

d. Birth certificate

e. EBT card

f. EPPICard
