A Subgrantee's written request for an appeal must include the
following details:
A. Subgrantee
Identification
1. The full legal name of the
Subgrantee;
2. If applicable, the
specific program or subgrant agreement under dispute should be clearly
identified.
B.
Description of adverse action(s);
1. A clear
and concise identification of the specific adverse action(s) the Subgrantee
seeks to dispute; and
2. A brief
explanation of how the adverse action negatively impacts the Subgrantee,
providing context for the appeal.
C. Representative's Signature
1. The signature of the authorized
representative filing the appeal on behalf of the Subgrantee.
2. The representative's title and clear
indication of their authority to act on behalf of the Subgrantee, if not
immediately apparent.
A Subgrantee's Organized Response shall include the following
elements:
A. Representative Contact
Information;
1. The full name and official
title of the representative handling the appeal.
2. Contact details, including a valid email
address and phone number, for all necessary communications regarding the
appeal.
B. Written
Appeal Explanation;
1. A clear and concise
written explanation detailing the basis for the appeal.
2. Specific facts, events, or circumstances
supporting the appeal, which clarify why the adverse action should be
reconsidered.
3. Relevant
references to regulations, policies, or procedures that substantiate the
Subgrantee's position.
C.
A table of contents;
1. A detailed table of
contents listing all supporting documents in the order they are
presented.
2. Page numbers must be
included for each document or section to allow for easy reference during the
review process.
D.
Supporting Evidence:
1. Copies of all relevant
documents or evidence that substantiate the Subgrantee's appeal.
2. Each document must be paginated and
organized in the same order as the table of contents to facilitate an efficient
and clear review process.
3. A
brief description or summary of each piece of evidence, explaining how it
supports the appeal.
