23 Miss. Code. R. 209-1.2 - Provider Enrollment Requirements
DME providers must satisfy all requirements set forth in Part 200, Chapter 4, Rule 4.8 in addition to the following provider type specific requirements:
A. National Provider
Identifier (NPI), verification from National Plan and Provider Enumeration
System (NPPES),
B. Provide written
confirmation from the IRS confirming the tax identification number and legal
business name,
C. Provide a copy of
current Medicare certification for the servicing location. Explanation of
Medicare Benefits (EOMB) is not acceptable, and
D. Provide a copy of DME or pharmacy permit
from MS State Board of Pharmacy for the servicing location.
