9 Miss. Code. R. § 7-2-2.12 - REPLACEMENT CERTIFICATES OF REGISTRATION AND AGENT PERMITS

If for any reason, a Certificate of Registration or Agent Permit must be replaced, the institution owner or agent must submit an appropriate affidavit form (Certificate of Registration Affidavit Form or Agent's Permit Affidavit Form) and replacement fee to the Commission on Proprietary School and College Registration. The fee for a duplicate Certificate of Registration or a duplicate Agent Permit is stipulated in the current Schedule of Fees (Appendix A).
