Nev. Admin. Code § 618.6316 - Application for permanent variance: General requirements
An application for a permanent variance from a standard adopted under chapter 618 of NRS must:
1. Be submitted on a form provided by the
Enforcement Section.
2. Be
legible.
3. Be signed by the
applicant or an authorized representative of the applicant.
4. Contain a statement, signed by a
representative of the employees or an employee who knows the contents of the
application, that he or she has read the application.
