Nev. Admin. Code § 645D.150 - Information required

In addition to the information required by NRS 645D.170, an application for a certificate must include the following information:

1. Two completed fingerprint cards of the applicant;

2. One recent photograph of the applicant; and

3. A document which proves that the applicant has:

(a) Error and omissions insurance of at least $100,000; and

(b) Liability insurance of at least $100,000.
