N.H. Admin. Code § He-P 301.08 - Procedures for Disclosure of Information
(a) Requests for
release of information under
RSA
141-C:10, shall be made to the commissioner
or his or her designee and shall describe the type, the purpose, and the
ultimate disposition of the requested information.
(b) In the case where the department receives
a report that a person, who works outside from his or her primary residence, or
who provides child care in his or her primary residence, or who prepares food
in his or her primary residence for sale to the public, is diagnosed with a
reportable disease, or with a condition that can pose a threat to the public
health, the following steps shall be taken:
(1) The department shall disclose to the
manager of the place of employment:
a. The
name of the individual employee so diagnosed;
b. The name of the reportable
disease;
c. The laboratory test
results associated with the reportable disease; and
d. What steps the manager shall take to
assure protection of the health of the public from exposure to the risks
associated with the reportable disease; and
(2) The personal identity of the employee
shall be kept confidential by the manager to whom a disclosure is made as
described in (1) above in accordance with
RSA
141-C:10, I.
