N.H. Admin. Code § Phy 404.06 - Supervisory Approval Letter
(a) For a physical
therapist holding a conditionally reinstated license, the letter indicating
supervisory approval shall:
(1) Be dated and
signed by the individual who is the physical therapist's most recent
supervisor;
(2) Be based on the
determinations by the supervisor required by
Phy
404.10;
(3) Make a statement that the physical
therapist is ready to practice under full licensure because the supervisor has
determined that they have the following requisite skills detailed in
Phy
404.10:
a. Evaluation
skills;
b. Treatment
skills;
c. Management skills;
and
d. Professional interaction
skills;
(4) State the
number of hours of direct personal supervision;
(5) State the number of hours of direct
supervision hours; and
(6) State
the number of general supervision hours.
(b) For a physical therapist assistant
holding a conditionally reinstated license, the letter indicating supervisory
approval shall:
(1) Be dated and signed by
the individual who is the physical therapist assistant's most recent
supervisor;
(2) Be based on the
determinations by the supervisor required by
Phy
404.11;
(3) Make a statement that the physical
therapist assistant is ready to practice under full licensure because the
supervisor has determined that the physical therapist assistant has the
following requisite skills detailed in
Phy
404.11:
a.
Assessment skills;
b. Treatment
skills;
c. Management skills;
and
d. Professional interaction
skills;
(4) State the
number of hours of direct personal supervision;
(5) State the number of hours of direct
supervision hours; and
(6) State
the number of general supervision hours.
