N.H. Admin. Code § Rec 403.03 - Reinstatement of Licenses Lapsed No Longer Than One Year
The board shall reinstate a license lapsed no longer than one year if the reinstatement applicant submits:
(a) A completed, signed, dated and notarized
"Application for Reinstatement of Licensure or Certification" form as described
in Rec
403.05;
(b) Proof in accordance with Rec 406 of
having completed 30 hours of maintenance of continuing competence for the
immediately preceding 2 years;
(c)
A photocopy of the applicant's currently valid NCTRC certificate or wallet
card;
(d) Payment of the
reinstatement fee set forth in
Ahp
301.02(a);
(e) A written statement that the
reinstatement applicant has not engaged in recreational therapy in New
Hampshire on a volunteer or paid basis since the date that his or her license
ceased to be valid; and
(f) Any
documentation required in
Rec
302.04 which was not previously provided to the
board.
