N.H. Admin. Code § Rec 403.04 - Reinstatement of Licenses Lapsed For Longer Than One Year
The board shall reinstate a license lapsed for more than one year if the reinstatement applicant:
(a) Is of good moral character, as evidenced
by:
(1) The answers to the "yes-no" questions
on the confidential and non-confidential parts of the reinstatement application
form described in Rec 403.05; and
(2) Any detailed reports of the relevant
circumstances related to answers to those questions;
(b) Is currently NCTRC certified;
and
(c) Submits the following to
the board:
(1) A completed, signed, dated and
notarized reinstatement application form as further described in
Rec
403.05;
(2) The supporting materials described in
Rec
403.06;
(3) Payment of the reinstatement fee set
forth in Ahp 301.02(a);
and
(4) Any documentation required
in Rec
302.04 which was not previously provided to the
board.
