N.H. Admin. Code § Ret 308.09 - Petition For Relief Due To Financial Inability To Pay

Petitions for relief pursuant to Ret 308.08 shall be filed in the form of a written statement, under oath, claiming financial inability to make such payments and disclosing all income, assets, and liabilities of the eligible employee and his or her spouse, if any.
