N.H. Code Admin. R. Cla 302.03 - Filing the Claim
The claimant shall file the claim by submitting:
(a) To the agency complained against, one
copy of the completed claim form and supporting documents; and
(b) To the secretary of state at the address
stated in (c) below:
(1) Three copies of the
completed claim form and supporting documents; and
(2) If the claim is for an amount greater
than $5,000.00, payment of a fee in the amount of $25.00.
(c) The filing address shall be:
