N.J. Admin. Code § 11:17-6.5 - Filing requirements
(a) No person, firm, association or
corporation shall act as an MGA with respect to risks located in this State for
an insurer licensed or domiciled in this State, unless the MGA and the insurer
have notified the Commissioner as provided (b) below.
(b) Insurers and MGAs required to notify the
Commissioner as provided at (a) above shall file a Notice of Managing General
Agent Contract in the form set forth as Form A in the Appendix to this
subchapter incorporated herein by reference by completing and jointly executing
the form and sending it to the Department at the following address:

(c) The Commissioner may refuse to file any
Notice of Managing General Agent Contract that is incomplete. The Commissioner
shall notify the insurer of a determination that the Notice is incomplete, and
the actions that must be taken or the items that must be submitted in order to
make a complete filing.
