N.J. Admin. Code § 17:1-7.8 - Employer resolution; involuntary disability application

(a) Applications for the involuntary disability retirement of an employee of a local employer must be accompanied by a resolution of the governing body, or in the case of a State employee, by a letter from the State department head, certifying that the employee is disabled and unable to perform the employee's regular or assigned duties.
(b) As 43:5-5 requires members to make their own option selection or beneficiary designation at retirement, an employer who is submitting an involuntary disability retirement application must select the maximum option and list the member's beneficiary as "estate". If the involuntary disability retirement is approved by the Board, the member may change the option selection and beneficiary designation for the pension benefit within 30 days. The member may also file a Designation of Beneficiary form, to designate a specific beneficiary to receive the member's life insurance benefits.
