A. An
operator shall file form C-117-A with the appropriate division district office
in accordance with Subsections B, C and H of
19.15.18.17 NMAC.
B. An operator shall file form C-117-B with
the division's Santa Fe office and the appropriate division district office in
accordance with Subsection D of
19.15.18.17
