N.Y. Comp. Codes R. & Regs. Tit. 11§ 27.2 - Excess line broker license application and renewal

(a) Each application for an excess line broker's license or renewal thereof shall be submitted to the Licensing Unit of the New York State Department of Financial Services located in Albany, NY, using the appropriate form, which may be obtained, upon request, from the Licensing Unit.

(b) All information called for by the forms shall be supplied thereon and shall be duly sworn to by the applicant.
