Ohio Admin. Code 124-9-05 - Questionnaires
(A) At any time
prior to a hearing, the board may order evidence taken in the form of
questionnaires.
(B) The board may
require questionnaires to be submitted at a hearing or
to be returned by mail
filed prior to the hearing.
(1) Questionnaires shall be treated as
admissions and may be completed with the advice of a party's
representative.
(2) All
questionnaires shall bear the oath or affirmation of the party completing
them.
(C) Questionnaires
may be used as the sole basis for deciding any appeal. Failure to respond to a
questionnaire may result in dismissal of the case.
