Ohio Admin. Code 3358:11-5-31 - Health/life insurance policy
(A) Health insurance. Medical, dental,
vision, and prescription drug insurance benefits are provided to full-time
employees. New full-time employees should apply for health insurance coverage
within the designated enrollment period from the start date of employment. For
full-time employees not covered by collective bargaining agreements, coverage
for medical, prescription, vision and dental benefits begins on the first day
of the month after the month of hire. Coverage for all benefits ends at the end
of the payroll period in which the employee ends employment at the college. A
bargaining employee shall refer to the respective collective bargaining
agreement.
(B) Life insurance. Each
full-time employee of the college is provided with a fully paid term life
insurance policy in an amount not less than fifty thousand dollars and not more
than two times the employee's regular full time salary, maximum cap of
three-hundred thousand dollars. Coverage is provided for the duration of
employment, within restrictions of the insurance policy itself.
