Ohio Admin. Code 4123-6-29 - Request for information by the treating provider
A provider treating an injured worker may, at any time, make a
request in writing, fascimile
facsimile, or
e-mail, or by telephone
in accordance with the bureau's confidentiality and
sensitive data requirements, for relevant information concerning
conditions, treatment or history for the claim. The request for information
shall be accompanied by an appropriate patient release of medical information.
A prompt response will be given to this request.
