The following federal statutes or regulations or state statutes and administrative rules make personal information maintained by the board confidential and identify the confidential personal information within the scope of rules promulgated by the board in accordance with section 1347.15 of the Revised Code:
(A)
Social security numbers:
5 U.S.C.
552a., unless the individual was told that
the number would be disclosed.
(B)
Records of
reporting required pursuant to
42 U.S.C. section
1320a-7e(b),
5 U.S.C. section
552a, and 45 C.F.R. part 61 for compliance
with the U.S. department of health and human services' healthcare integrity and
protection data bank (HIPDB).
(C)
"Bureau of
criminal identification and investigation" criminal records check results:
section 4776.04 of the Revised
Code.
(D)
Records required or allowed to be kept confidential
pursuant to section 149.43 of the Revised
Code.
(E)
Information and records received or generated by the
board pursuant to an investigation: division (E) of section
4755.02 and division (B) of
section 4779.33 of the Revised
Code.
(F)
Medical records submitted with requests for testing
accommodations and/or continuing education waiver requests: 5 C.F.R. 164
(2014).
(G)
College and university transcripts:
20 U.S.C.
1232 g(2013).
