Ohio Admin. Code 4757-5-11 - Standards of ethical practice and professional conduct: change of name and/or address
(A)
All licensees or registrants are required within ninety
forty-five days
to notify the board of any changes of name or mailing address. Failure to do so
may result in disciplinary action by the board.
(B) All applicants, licensees, or registrants
must furnish
provide to the board, and maintain, an electronic mail
(e-mail) address for the purposes of licensure system access.
