(A) "Courage to be Safe: Sixteen Life Safety Initiatives Course" or "Courage to Be Safe" course (March 2020) is based on the "National Fallen Firefighters Foundation's" "Everyone Goes Home Program" and is a component of the Foundation's "16 Firefighter Life Safety Initiatives." (B) "Emergency Vehicle Operations Course Packet" or "EVOC Packet" or form "EMS 0210" (August 2019) is a course consisting of a minimum of sixteen hours that focuses on familiarizing participants with terminology, driving skills, and liability issues that relate to the operation of a fire department vehicle. The course objectives, established by the executive director, have been designed to meet, and shall be consistent with, the general knowledge requirements, general skill requirements, and job performance requirements specified in "NFPA 1002: Standard for Fire Apparatus Driver/Operator Professional Qualifications" and "NFPA 1451: Standard for a Fire and Emergency Service Vehicle Operations Training Program." The course is a qualification requirement for Ohio firefighter I and firefighter II certification. (C) "Exemption Request for Continuing Education for Fire Certificates" or form "EMS 1103" (January 2018) is made available to certificate holders who wish to request that the executive director waive part of or all of the continuing education or instructional requirements needed to renew. This form can be accessed at the division's forms and applications website at http://www.ems.ohio.gov/forms.aspx.

(D) "Fire and Emergency Services Instructor I & II Course Packet" or form "EMS 0122" (October 2019) includes the fire and emergency services instructor course overview, course admission requirements, qualifications for certification, course objectives, and recommended hours guide. The packet is made available to students, instructors, and program directors and can be accessed at the division's fire charter and EMS course materials website at https://ems.ohio.gov .

(D)(E) "Fire and Emergency Services, Assistant Fire, or Fire Safety Inspector Instructor Initial Application" or form "EMS 0107" (April 2018) is the application for a fire instructor, assistant fire instructor, or fire safety inspector certification fire and emergency services instructor I, or fire and emergency services instructor II, certification issued by the executive director. (F) "Fire and Emergency Services, Assistant Fire, Assistant Fire, Fire Safety Inspector, or Live Fire Instructor Renewal Application" or form "EMS 1102" (January 2018) is made available to a holder of a certificate to teach who wishes to renew the certificate to teach and can be accessed at the division's renewal website at https://services.dps.ohio.gov/EMSProviders.

(G) "Fire Charter Application" or form "EMS 0029" (January 2018) is made available to a training institution that wishes to apply for a fire charter issued by the executive director and can be accessed at the division's forms and applications website at http://www.ems.ohio.gov/forms.aspx.

(H) "Fire Charter Renewal Application" or form "EMS 0027" (January 2021) is made available to existing fire charters issued by the executive director and can be accessed at the division's forms and applications website at https://www.ems.ohio.gov .

(G)(I) "Fire, Fire Safety Inspector, and/ and Emergency Services Instructor, or Live Fire Instructor Reinstatement Application" or form "EMS 0109" (January 2018) is made available to individuals wanting to regain a fire and emergency services instructor, or live fire instructor, or fire safety inspector instructor certificate that has expired, been voluntarily surrendered, or been revoked and can be accessed at the division's forms and applications website at http://www.ems.ohio.gov/forms.aspx.

(H)(J) "Fire and Emergency Services Instructor Reciprocity Application" or form "EMS 0098" (January 2022) is made available to an applicant who wishes to be issued a fire and emergency services instructor certificate by the executive director, based on training received from another state or a branch of the United States military. Division staff mail form "EMS 0098" to a potential applicant following that person' ' s completion and the division's review of the "Fire Instructor Reciprocity Packet" or form EMS 5918, which can be can be accessed at the division's forms and applications website at http://www.ems.ohio.gov/forms.aspx.

(I)(K) "Fire and Emergency Services Instructor Reciprocity Packet" or form "EMS 5918" (January 2022) is made available to a potential applicant for a fire and emergency services instructor certificate issued by the executive director, to provide documentation of training received from another state or a branch of the United States military. (J)(L) "Fire Safety Inspector Course Packet" or form "EMS 0226" (January 2020) includes the fire safety inspector course overview, course admission requirements, qualifications for certification, course objectives, and recommended hours guide. (Q) "Firefighter, Fire Safety Inspector and/or Hazard Recognition Officer Reinstatement Application" or form "EMS 0116" (January 2018) is made available to an applicant seeking reinstatement of a firefighter, fire safety inspector, or hazard recognition officer certificate that has expired, been voluntarily surrendered, or been revoked and can be accessed at the division's forms and applications website at http://www.ems.ohio.gov/forms.aspx.

(R) "Firefighter, Fire Safety Inspector, or Hazard Recognition Officer Initial Application" or form "EMS 0053" (January 2018) is the application for a firefighter, fire safety inspector, or hazard recognition officer certification, issued by the executive director. The application is made available through an online portal accessed at chartered fire training programs by an applicant who has been verified by a program director as meeting all training requirements to apply for a firefighter, fire safety inspector, or hazard recognition officer certification.

(S) "Firefighter, Fire Safety Inspector, or Hazard Recognition Officer Renewal Application" or form "EMS 1104" (January 2018) is made available to a holder of a firefighter, fire safety inspector, or hazard recognition officer certificate who wishes to renew the certificate and can be accessed at the division's renewal website at https://services.dps.ohio.gov/EMSProviders.

(T) "Firefighter Reciprocity Application" or form "EMS 0081" (January 2022) is made available to an applicant who wishes to be issued a firefighter certificate by the executive director, based on training received from another state or a branch of the United States military. Division staff mail form "EMS 0081" to a potential applicant following that person's completion and the division's review of the "Firefighter Reciprocity Packet" or form EMS 5917, which can be can be accessed at the division's forms and applications website at http://www.ems.ohio.gov/forms.aspx.

(U) "Firefighter Reciprocity Packet" or form "EMS 5917" (January 2022) (November 2022) is made available to a potential applicant who wishes to be issued a firefighter certificate by the executive director, to document training received from another state or a branch of the United States military. The packet is available to applicants seeking reciprocity and can be accessed at the division's forms and applications website at http://www.ems.ohio.gov/forms.aspx.

(V) "Hazardous Materials Awareness and Operations Course Packet" or form "EMS 0205" (January 2020) is a course consisting of a minimum of sixteen hours that is approved by the chartered program and is consistent with the requirements of "NFPA 1072" related to the minimum requirements in terms of professional core competencies for hazardous materials awareness and operations and can be accessed at the division's forms and applications website at http:www.ems.ohio.gov/forms.aspx.

(W) "Hazard Recognition Officer Course Packet" or form "EMS 0311" (January 2020) sets forth the admission requirements, qualifications for certification, course objectives and recommended hours, and knowledge and skill objectives required to be taught in a hazard recognition officer course, as approved by the executive director. (X) "Live Fire Instructor Initial Application" or form "EMS 0125" (January 2018) is made available to applicants who want to apply for apply for an initial live fire instructor (LFI) certificate to teach and can be accessed at the live fire instructor section of the division's forms and applications website at http://www.ems.ohio.gov/forms.aspx by using the exemption link.

(Y) "Live Fire Training Awareness Module" or form "EMS 1110" (October 2019) is a component of the training required to be certified as a fire instructor. The module provides an overview of "NFPA 1403: Standard on Live Fire Training Evolutions" and reviews the aspects that fire instructors must consider when conducting live fire training evolutions in a safe and compliant manner. The module presents information on legal considerations, training preparation, and building a culture of safety in fire service training programs. (Z)(Y) "Live Fire Training-Operations Level Course" "Live Fire Instructor Course Packet" or form "EMS 1107" (March 2021) is a course consisting of a minimum of twenty- four hours approved by the executive director that provides students with the necessary knowledge and tools to prepare and conduct live fire training evolutions. The course includes an in-depth analysis of "NFPA 1403: Standard on Live Fire Training Evolutions," addressing evolutions at acquired structures; gas-fired training buildings; non-gas fired training buildings; and exterior props, vehicle fires, and exterior class B fires. The course is required for certification as a "live fire instructor."
