Okla. Admin. Code § 595:65-1-3 - Collision report instructions
The investigating agency shall use the latest version of the "Official Oklahoma Traffic Collision Report Instruction Manual" when completing and submitting a collision report. The latest version of the instruction manual is available from the Department of Public Safety on its website at https://oklahoma.gov/content/dam/ok/en/dps/docs/oklahoma_collision_report_form_instruction_manual_updated_5-8-2019_otcrim2011.pdf.
