Okla. Admin. Code § 710:1-3-2 - Required forms and where available

(a) The Oklahoma Tax Commission utilizes a wide variety of forms in the administration of the tax laws.

Oklahoma tax forms may be accessed through the Tax Commission website at www.tax.ok.gov.

(b) The Tax Commission may require any person filing a report or return required by the provisions of any state tax law to file the report or return by electronic means or may allow, upon a written request, a taxpayer to file a return on paper that is required to be filed electronically.
