Or. Admin. Code § 255-040-0040 - Personal Review Packets

The Personal Review Packet shall contain:

(1) Institution face sheet;

(2) All Board Action Forms since the prison term hearing;

(3) Psychological evaluations (last 6 months);

(4) Recommendation to reset the release date to an earlier date;

(5) Correspondence;

(6) Field parole analysis report or report of similar content; and

(7) Court orders.
