Or. Admin. Code § 333-536-0035 - Return of Agency License

Each license certificate in the licensee's possession shall be returned to the Authority immediately upon the suspension or revocation of the license, failure to renew the license by the date of expiration, or if operation is discontinued by the voluntary action of the licensee.
