Or. Admin. Code § 411-055-0355 - Admittance Criteria
Intensive Intervention Services (IIS) may be provided to individuals who meet all the following requirements:
(1) The individual is at least age 18 prior
to admission.
(2) The individual
has been assessed by the IDT as needing Intensive Intervention Services during
the person-centered service planning process.
(3) An individually-based limitation,
specific to the individual, has been justified and documented in the
individual's person-centered service plan by the person-centered service plan
coordinator, as outlined in OAR
411-004-0030.
