Or. Admin. Code § 411-071-0027 - Confidentiality of Assessment Information
(1) Any records, forms, or information
collected during the assessment process that identify an individual by name or
address must be confidential and subject to the Department's rules on
confidentiality set forth in OAR chapter 411, division 005.
(2) Certified programs must not release
information obtained during the assessment process to any person or entity not
authorized by law to receive such information without the written consent of
the individual or the individual's legal guardian.
