Or. Admin. Code § 415-065-0050 - Unique Identification Number
(1) The program shall assign a unique
licensee identification number to each licensee enrolled in the program:
(a) If the program is operated by a
contractor, the contractor and the Division shall use the same number and shall
include the number in any communications or data exchanges involving the
licensee;
(b) Once a unique
identification is used the program may not use it again for any other licensee
enrolled in the program and it shall be retired when the licensee is no longer
enrolled in the program; and
(c)
If a licensee reenrolls in the the program the program must use the same unique
identification number that was previously assigned to the licensee.
(2) The program may not use all or
a portion of a licensee's social security number as the unique identification
number.
