Or. Admin. Code § 582-115-0020 - Refusal to participate in a required activity
(1) When the individual refuses to
participate in a required activity, documentation of that refusal shall be
provided within 10 calendar days and shall at a minimum contain:
(a) Name of the individual;
(b) Description of the refusal including
reason for refusal;
(c) Signature
of the individual or individual's representative;
(d) Signature of Program personnel
documenting the individual's refusal;
(e) Date of signatures; and
(f) Date and method of transmittal of
documentation of refusal to the individual.
(2) The Program shall retain a copy of all
documentation in a manner consistent with the Program's case management
system.
