Or. Admin. Code § 735-050-0005 - Collision Reporting to DMV
A person required to submit a collision report under ORS 811.720 must complete and submit either of the following reports to DMV:
1) An Oregon Traffic Collision and Insurance
Report (DMV form 735-32); or
2) A
DMV2U eCollision Report located on DMV's website (https://dmv2u.oregon.gov/eServices/_/).
