Or. Admin. Code § 812-007-0130 - Renewal of Lead-Based Paint Activity Licenses - Individuals
Individuals licensed under these rules may renew their licenses by submitting the following:
(1) A
properly completed application for license renewal on a form provided by the
board;
(2) The fee established in
OAR 812-007-0160; and
(3) Proof that the individual is certified by
OHA.
