Or. Admin. Code § 812-030-0200 - General Application Requirements
An individual must submit the following to qualify for a locksmith certificate:
(1) An
application on a form provided by the board;
(2) The fee established in OAR
812-030-0250; and
(3) If applicable, the CCB license number of
the business owned by or employing the applicant.
