Or. Admin. Code § 836-052-0185 - Reporting of Multiple Policies
(1)
On or before March 1 of each year, each issuer shall report to the Director the
following information for every individual resident of this state for which the
issuer has in force more than one Medicare supplement insurance policy or
certificate:
(a) Policy and certificate
number; and
(b) Date of
issuance.
(2) The
information required under section (1) of this rule must be grouped by
individual policyholder.
(3) Each
issuer shall report the information required under this rule on the reporting
form prescribed in Exhibit 1 to this rule.
