31 Pa. Code § 25.15 - Forms-additional information and exhibits
(a) In addition to the information expressly
required to be included in Forms A-F located in Appendix A, the person filing
shall provide further material information, if any, as necessary for the
completion or clarity of the information expressly required in the form. The
person filing may also file exhibits as desired in addition to those expressly
required by the form. The exhibits shall be marked to indicate clearly the
subject matters to which they refer. The Commissioner may require the person
filing the form to provide additional information as may be necessary to
determine compliance with the act.
(b) Changes to Forms A-F must include on the
top of the first page the phrase "Change No. (insert number) to" and indicate
the date of the change and not the date of the original filing.
