(a) An
applicant for licensure by endorsement who is educated in another state or
territory of the United States is required to submit the professional
credentials in §
40.11 (relating to license by
examination; requirements for examination), and comply with the following:
(1) The applicant shall submit a written
application on forms provided by the Board, together with evidence satisfactory
to the Board that the applicant is licensed or otherwise registered as a
physical therapist in another state or territory of the United States, or in
the District of Columbia, if the requirements for licensure or registration in
the state, territory or district were, at the date of licensure or
registration, substantially equal to the requirements for licensure or
registration in this Commonwealth under the act.
(2) The applicant shall submit the fee
prescribed in §
40.5 (relating to fees) for
licensure by endorsement.
(3) The
applicant shall have completed at least 3 hours of approved training in child
abuse recognition and reporting in accordance with §
40.208(a)
(relating to child abuse recognition and reporting-mandatory training
requirement).
(b) In
addition to the requirements in subsection (a), an applicant for licensure by
endorsement who received education in a country outside of the United States is
required to meet the educational requirements as set forth in §
40.17(5) and (6)
(relating to foreign-educated physical therapists applying to take the
licensure examination and pursue a clinical experience).
