52 Pa. Code § 64.103 - Medical certification
Certifications initially may be written or oral, subject to the right of the LEC to verify the certification by calling the physician or to require written confirmation within 7 days. All certifications, whether written or oral, shall include all of the following information.
(1) The name, address and telephone number of
the customer in whose name the account is registered.
(2) The name and address of the afflicted
person and the afflicted person's relationship to the customer.
(3) The nature and anticipated length of the
affliction.
(4) The name, office
address and telephone number of the certifying physician.
(5) The specific reason why access to
telecommunications service must be maintained.
