58 Pa. Code § 165.173 - Complaints
(a) Complaints,
appeals, affidavits, petitions, motions and other formal papers shall be filed
with the Commission at its executive offices in Harrisburg,
Pennsylvania.
(b) The same shall be
legible and in paragraph form and shall contain all principal allegations of
fact and rules or issues of law believed to be relevant. Complaints, appeals,
original petitions or similar motions shall be sworn to by the
applicant.
