A. Copies of the following authorized forms are available on the Rhode Island Department of State Office of the Secretary of State website at www.sos.ri.gov or as otherwise indicated herein.

1. The forms authorized in R.I. Gen. Laws § 6A-9-521(a) and (b) and any amendments thereto;

2. The Rhode Island state-approved form for information requests; and

3. The most current International Association of Commercial Administrators (IACA) national filing forms, which may be found on the internet at www.iaca.org.
