the THSA Coordinator, or whenever requested by one of the members. The

of interest provision for committee members. Any committee member who has, will
have, or later acquires an interest, direct or indirect, in any transaction

of such actual or prospective interest. Upon such disclosure, this member shall
not participate in any deliberations or actions by the committee that directly
or indirectly affects that transaction or interest.
