16 Tex. Admin. Code § 112.74 - Continuing Education Provider-Issuance of Registration
(a) The department will issue an applicant,
whose application has been approved, a registration containing the registered
provider's name, registration number, and expiration date.
(b) A continuing education provider
registration issued by the department remains the property of the
department.
(c) The department will
issue a duplicate registration upon written request by the provider, in a form
and manner prescribed by department, and upon payment of the
duplicate/replacement fee under §112.110.
