22 Tex. Admin. Code § 161.30 - Registration and Renewal
(a) In
accordance with Chapter 156 of the Act, a physician license must be registered
with the board and renewed every two years after it is issued.
(b) A renewal notice will be sent to the
physician's address of record at least 60 days prior to the expiration date of
the registration.
(c) The physician
must:
(1) complete the renewal form;
(2) pay the renewal fee and any additional
fees, as applicable:
(A) initial biennial
permit - $456.00;
(B) subsequent
biennial permit - $452.00;
(3) verify and update their physician
profile; and
(4) provide any other
relevant information requested.
