26 Tex. Admin. Code § 264.239 - Documentation of Services Delivered
(a) An
employer or DR must ensure that documentation of services delivered includes:
(1) each element required by an individual's
program; and
(2) service dates
within the same calendar month.
(b) In addition to documentation generated by
an EVV system, documentation of services delivered must include:
(1) time sheets for employees who are not
required to document their time worked using an EVV system as required by
Chapter 68 of this title (relating to Electronic Visit Verification (EVV)
System);
(2) time sheets or
invoices for contractors;
(3)
invoices for vendors;
(4) receipts
when payment has been made for a service; and
(5) other documentation in accordance with
requirements of the individual's program.
(c) Unless using an EVV system as required by
Chapter 68 of this title:
(1) an employer or
DR must review documentation of services delivered and obtain corrections or
revisions before submitting the document to the CDSA for payment;
(2) the person making an error or omission on
documentation of services delivered must:
(A)
enter the omission; and
(B) for an
error, make correction by:
(i) making one
line through the error;
(ii)
entering the correction; and
(iii)
initialing and dating the correction; and
(3) the employer or DR must sign and date the
documentation of services delivered after the last entry or correction made by
the service provider to signify approval for payment of the documentation of
services delivered.
